We Can’t Build the NHS of the Future with Yesterday’s Leadership Tools

There’s a line in the NHS’s new 10-year health plan that sticks with me:
“Healthcare work will look very different in 10 years’ time — and we will need a very different kind of workforce strategy.”
It’s not just a headline. It’s a signal of a deeper truth: transformation can’t happen without people.
The future of healthcare won’t be built by infrastructure alone. It’ll be shaped by the capability, wellbeing, leadership, and lived experiences of the people inside it. And if we want to deliver prevention over crisis, neighbourhood care over hospital dependency, and digital-first access that still feels human, we need to radically shift how we support, lead, and grow our workforce.
At Boo Coaching & Consulting, this is the space we’ve always lived in — supporting leaders and organisations to thrive through change, complexity, and compassion. So when I read this plan, it didn’t feel like a new direction — it felt like the NHS finally catching up with the human truth we’ve seen for years:
You can’t pour from an empty cup. You can’t drive transformation with exhausted people. And you can’t build a thriving system without brave, brilliant leadership.
What the plan gets right
There’s a lot to be hopeful about in Fit for the Future. The commitments go beyond recruitment numbers and hospital beds. They reach into the fabric of the workplace — the culture, the support, the expectations.
Here’s what stood out:
- Every NHS staff member will have a personalised career development and coaching plan
- AI will act as a supportive tool, freeing clinicians to spend more time on people, not paperwork
- Managers will have real autonomy to lead, appraise, reward, and tackle underperformance
- A new College of Executive and Clinical Leadership will be established to raise the bar for system-wide leadership
- A big shift away from over-reliance on international recruitment, towards growing local talent and widening access
This is not small stuff.
This is cultural change at scale.
And it’s long overdue.
But culture doesn’t shift just because a policy says so.
This plan talks a lot about empowerment, prevention, and personalised development — and I’m here for it. But we have to make sure those ambitions don’t stay as bullet points in a strategy. They need to be lived, breathed, and embedded across every team, service, and system.
